Filling in and sending this form demonstrates your acceptance of the rules of The-DHN and your willingness to abide by them.

Completed forms are processed on a weekly basis so we ask for your patience. Our Recruiter will place you with a Group Leader who will contact you with your first case and an introduction to the other volunteer healers in your Group. The-DHN Web Site Manager will also tell you when your email address has been added to DHNnotices, our Mailing List. We endeavor to keep The-DHN as friendly and personal as possible. You will be made to feel most welcome.

If you have any concerns about the use to which your personal information is put, please read our Privacy Policy. None of the information requested is obligatory, with the exception of a name and a valid email address, without which we cannot contact you and the Network fails.
